Full-Arch Dental Rehabilitation

The All-on-4® treatment concept is a surgical restorative technique designed to provide a fixed, full-arch teeth replacement for patients with missing or failing teeth. This approach prioritises a collaborative planning process to develop a treatment path tailored to your unique clinical needs.

Utilising advanced surgical protocols, the All-on-4® concept aims to provide a functional restorative outcome in a condensed timeframe, often within 24 to 48 hours of the surgical procedure, where clinically appropriate.

Our laboratory team collaborates with you and your clinician to determine the appropriate shade and morphology of your provisional restoration. This ensures the design complements your oral anatomy and functional requirements.

  • Clinical Photography: To document and plan your facial aesthetics.
  • Digital or Physical Impressions: To create an accurate map of your oral structure.
  • Aesthetic & Functional Consultation: To discuss your preferences for tooth shade and shape within clinical parameters.

The surgical phase involves the removal of compromised teeth and the clinical preparation of the site for implant placement. Following the procedure, you will be fitted with a provisional set of teeth. These functional restorations are worn for approximately 3–6 months while the implants integrate with the bone (osseointegration), allowing us to refine speech, function, and aesthetics for your final bridge.

  • Anaesthesia & Facilities: Procedures are conducted under local anaesthetic or general anaesthesia in a clinical environment.
  • Surgical Site Preparation: Removal of indicated teeth and necessary bone contouring.
  • Provisional Restoration: Indexing and insertion of the initial fixed bridge, typically within 24 hours of surgery.

Following surgery, you will attend a series of scheduled review appointments. These consultations allow our clinical team to monitor the healing process (osseointegration) and evaluate the function and aesthetics of your provisional restoration. Your feedback during this phase is used to refine the design of your final bridge.

  • Clinical Healing Reviews: Scheduled at 2 weeks, 1 month, 2 months, and 3 months.
  • Master Impressions: Taken once the soft tissues have stabilised and the implants have integrated.
  • Final Bridge Try-in: A clinical assessment of the permanent restoration to ensure optimal fit, bite, and appearance before final insertion.

During this final stage, your bridge is clinically fitted and secured. This restoration is designed to provide long-term durability, supporting optimal dental function, speech, and a natural appearance that complements your individual features.

  • Permanent Bridge Fitting: Replacement of the provisional restoration with the final, reinforced bridge.
  • Functional Assessment: Final verification of your bite (occlusion) and speech.
  • Long-term Maintenance Plan: Establishing a professional hygiene schedule to support the health of your implants.

FAQs

What Are All-On-4® Dental Implants?

The All-on-4® treatment concept is a surgical restorative solution for patients who currently use removable dentures or are facing the loss of their natural teeth. By utilizing four Nobel Biocare titanium implants to support a fixed full-arch bridge, this technique provides a stable alternative to traditional removable prostheses.

This treatment is designed to provide a natural-looking result that aims to support dental function and speech. Because the bridge is fixed, it avoids the movement associated with traditional dentures, allowing patients to eat a wider variety of foods once the healing process is complete.

One of the primary benefits of the All-on-4® concept is the streamlined surgical protocol, which in many cases allows for the attachment of a provisional bridge within 1–2 days of surgery. By tilting the posterior implants, the procedure often utilizes existing bone volume, which may reduce the necessity for extensive bone grafting in eligible patients.

How Can I Regain My Smile With All-On-4®?

When natural teeth are missing or severely compromised, it can impact essential functions such as chewing and speech. If left unaddressed, tooth loss can lead to further clinical complications, including bone resorption and changes to the oral structure.

Historically, full-arch replacement involved removable dentures or multiple individual implants which, in some cases, required supplementary procedures like bone grafting.

The All-on-4® treatment concept offers a streamlined surgical alternative. This protocol utilizes four titanium implants to support a fixed bridge, often utilizing existing bone volume to minimize the need for grafting in eligible patients. In many cases, a provisional restorative bridge can be fitted within 24–48 hours of the surgical procedure, allowing for an earlier return to dental function during the healing phase.

Do I Need a Bone Graft for All-On-4®?

Traditional dental implant techniques often require sufficient bone volume, which may necessitate supplementary procedures such as bone grafting if significant bone loss has occurred.

The All-on-4® treatment concept utilizes a specific surgical protocol—tilting the posterior implants—to maximie the use of available bone. This often reduces the requirement for bone grafting, making it a viable option for many patients who have experienced bone resorption.

For patients with more advanced bone loss, our clinical team can assess suitability for alternative implant techniques designed to utilize different areas of the jawbone. To determine if this approach is appropriate for your clinical needs, we recommend a comprehensive assessment with our dental practitioners.

  • Educational Information Sessions: We offer sessions to help patients understand the clinical aspects of the All-on-4® protocol.
  • Clinical Consultation: A thorough examination is required to develop a tailored treatment plan.
